FMCSA Medical Certification Changes

What is the FMCSA Medical Examiner’s Certification Integration Rule?
Starting June 23, 2025, Certified Medical Examiners must send results of DOT medical exams directly to the National Registry of Certified Medical Examiners.
The FMCSA will then electronically transmit those results to state licensing agencies, eliminating the need for commercial drivers to submit paper certificates themselves.
Some states are already implementing the Integration rule process.
What Does the Integration Rule Mean for Employers?
Employers must continue checking that their drivers are deemed qualified by a Certified Medical Examiner and include a note in the driver qualification file verifying that the examiner is listed in the National Registry.
CDL Drivers
- Run a medical certificate MVR to verify the driver's record is updated with the new medical qualification.
- Place the digital medical certificate in the driver's qualification file.
Non-CDL Drivers
- Ensure the driver carries the physical medical certificate on their person while driving.
- Obtain a physical copy of the medical certificate and keep it in the driver's qualification file.
