This form is used to document the reasons for conducting Reasonable-Suspicion testing including the signs and symptoms of alcohol misuse and/or drug use. The documentation should relate to an individuals appearance, behavior, speech and body odors. This document must be signed by the supervisor or trained company official who made the observations and completed within 24 hours or before the test results are released, whichever comes first.
