If you have an LCV driver who meets the Grandfathering requirements under Part 380.111, you may complete and sign this form for verification. Retain this documentation in the drivers file. Such a driver would not be required to complete LCV training as he/she has shown evidence of having an appropriate background and experience for safely operating such vehicles.
