This certificate can be used to document a drivers completion of a qualified LCV Driver Training Course. All double and triple trailer drivers with less than two years of experience must complete LCV training in compliance with Part 380 or be issued a Certificate of Grandfathering. Complete this form to certify that the driver completed the necessary training. Retain a copy in the drivers file and provide a copy to the driver for his/her own records.
