A driver who first began operating a commercial motor vehicle requiring a Commercial Drivers License (CDL) in interstate commerce after July 20, 2003 must receive Entry-Level Driver Training. Use this form to document a drivers completion of this mandatory training. Provide the driver with a copy and maintain a copy in his/her Driver Qualification File.
