This form can be used by an employer or DER (Designated Employer Representative) to verify that an individual sent for a drug or alcohol test has arrived at the collection or testing site in a timely manner. An employees failure to arrive at a testing or collection site in a timely manner could constitute a Refusal-to-Test situation.
